2. 2. 3 Passport size photogrpahs of each parent
3. 3. Copies of Aadhar card of both parents and child.
4. 4. Family Id, Birth Certificate, SLC, Medical report of the child.
5. 5. 2 Photos each of the persons you want to authorize for pick up.
