Documents Needed

1. 1. 6 Passport size photographs of the child.
2. 2. 3 Passport size photogrpahs of each parent
3. 3. Copy of Aadhar card of both parents and child.
4. 4. Family Id, Birth Certificate, SLC, Medical report of the child.
5. 5. 2 Photos each of the persons you want to authorize for pick up.
6. 6. Signature of both parents in this form is mandatory.
